What does Exclusion mean?

A service, item, or circumstance the plan document says it does not cover.

Illustration — not a plan price

An excluded service can remain the member's responsibility and may not count toward the deductible or out-of-pocket maximum.

The actual plan document controls. Verify its definitions, network, exclusions, dates, and amounts before relying on the illustration.

Definitions here are never for sale — no plan is recommended on a comprehension page.
