What does POS Plan mean?

A plan type that combines network rules, primary-care coordination, and some out-of-network coverage.

Illustration — not a plan price

A POS plan may require a referral for specialist care and apply different cost sharing outside the network.

The actual plan document controls. Verify its definitions, network, exclusions, dates, and amounts before relying on the illustration.

Definitions here are never for sale — no plan is recommended on a comprehension page.
